Registering with Disability Services
You must first register with Disability Services to access support.
We recommend you register as soon as you can. This is because setting up disability support can take several months.
How to register:
1. Step 1: Complete our quick and easy sign up form
2. Step 2: Provide supporting information about your disability
More about your supporting information:
What type of supporting information do I need?
The information required depends both on your disability and the level of support you are requesting.
Supporting information needed for specific learning difficulties, including dyslexia and dyspraxia
The University of Leeds supporting information form
The best way to gather the correct information is to ask your GP or other medical professional to complete our supporting information form.
[Please note, this form can’t be used for Specific Learning Difficulties like dyslexia and dyspraxia. This is because these conditions cannot be diagnosed by a doctor].
How do I check that the information I have is suitable?
Our handy checker will tell you the types of support you could access with the information you have. You may find it useful to check this first.
Don’t forget: your supporting information must:
- Be on official headed paper
- Include identifying details such as your name or date of birth
- Be signed by a medical professional, or someone acting on their behalf
- Be as up to date as possible
- Give a clear clinical/medical diagnosis of a disability that is likely to affect your ability to do everyday or academic tasks
- Clearly state that your disability is long-term (i.e. has lasted, or is likely to last, for at least 12 months)
- Indicate the impact your disability has on your daily life, particularly your studies
- Be written in English or be a formally certified translation
